Closed 13 Jul 2018

Opened 29 May 2018

Overview

In line with changes made to privacy legislation by the General Data Protection Regulation and the Data Protection Act 2018, we need to confirm that you still want to remain on our Surrey Learning Disability Partnership Board distribution list and continue to receive information from Surrey County Council (who help share information with network members).

Why your views matter

This online form is for our professional members only (e.g. service providers, health colleagues and Surrey County Council staff).  By completing this form, you can choose to be part of our email list.

What happens to your information?

  • Your name and contact details are kept on our secure confidential database. This information is not shared with anyone or any organisation without your specific agreement.
  • We use your contact details to send you information about the Surrey Learning Disability Partnership Board meetings (including agenda and minutes), information on learning disability matters and opportunities/events that may be relevant to you.

To allow us to do this and make sure we follow the law, please answer the following online survey.

Alternatively you can contact Sarah Linington with your preference by any of the following methods:

  • Post: Sarah Linington, Room 34, ASC Information and Engagement Team, Surrey County Council, County Hall, Penrhyn Road, Kingston upon Thames, KT1 2DN
  • Email: asc.engagement@surreycc.gov.uk
  • Telephone/SMS text: 0781 6077122

If we haven’t heard back from you by 13 July 2018, we will have to assume that you no longer wish to be on the Surrey Learning Disability Partnership Board distribution list and will therefore remove you from our database.

Please note that it is your responsibility to notify us about any changes to the personal details you provide.
