Overview

Dear parent / carer,

Surrey County Council are collecting the views of people that have used the CAMHS Community Nursing service, in order to understand if the service is performing well and meets people’s needs.  The CAMHS Community Nurses provide therapeutic advice and support to children and young people who are experiencing mild to moderate Emotional Behaviour and Mental Heath difficulties. The service is delivered in schools and in local communities.

You are not obligated to take part in this survey, but we would be very grateful if you could take 5 minutes to complete this form with your child.

The survey closes on 18 July 2016.

The information you give us will be compared to that of other children and young people to help us make improvements to the service. Please be assured that personal information will not be shared with anyone else, except in rare circumstances where the information you give us reveals concerns about your child’s safety.
