The Health & Wellbeing Commissioning Team at Surrey County Council is reviewing Short Breaks services to inform their future design.
We have recently launched an in-depth survey of parents and carers who are accessing Short Breaks services funded by Surrey County Council. These include schemes run by

This survey is about all the other schemes and activities in the community that families use.
We would be grateful if you could complete this survey for each community service - other than Surrey County Council Short Breaks services - that is used by your child(ren) with additional needs & disabilities and provide some basic information about your child(ren) so that we can understand the needs and age groups that different activities serve. This will give us great insights into the other services families use, and why they use them, which will inform the future recommissioning of our services.
Please note, we will only use the information provided to make improvements to Short Break services and to the information available to families, and your personal information will not be shared with anyone at Surrey County Council.
