CYP Haven Review - Children and young people's questionnaire

Overview

The CYP Haven is a mental health crisis drop in service for young people aged 10-18 years old. There are 4 across Surrey in Guildford, Epsom, Staines and Redhill.

We are looking to get your views on the Children and Young People's Haven. This questionnaire can be answered by any child or young person, whether you have been to the CYP Haven or have not heard of it. Please just skip past any questions you can not answer.

Why your views matter

We are reviewing the CYP Havens to gather feedback from children and young people, parents and professionals. This is to gain an insight into how the service is currently used and to understand if any changes or improvements can be made. This will enable us to provide the best service for Children and young people across Surrey.
