WE’D LIKE YOUR FEEDBACK ON THE MAX CARD!

In April 2016 Surrey Short Breaks introduced the Max Card; a brand new initiative providing families registered on the Surrey Children’s Disability Register with a discount card to be used at a range of play and leisure settings across the UK. Over 2000 families in Surrey with disabled children have now been issued with a Max Card.

Please take a few minutes to tell us about your experiences of the Max Card to help us evaluate the scheme and plan for the future. We are keen to find out if and how the card is benefitting you and your family.
