Recognising You Awards 2022 - Nomination Form

Closes 16 Oct 2022

Opened 15 Jul 2022

We want to hear the great achievements of Surrey's children and young people aged 5-25.

This year, the User Voice and Participation Team are facilitating the Recognising You Awards 2022.

We want to recognise the wonderful achievements of Surrey’s children and young people and have the opportunity to honour and congratulate them with an awards ceremony. We need your help to do this! We would like to ask you to nominate children and young people for a Recognising You Award!

We believe that every single child and young person deserves recognition for their accomplishments; this could be a fantastic achievement at school, progress that’s been made or obstacles that have been overcome.

You're welcome to nominate more than one young person, in fact, we encourage it!! However you will need to complete the survey once per nomination. Once you have submitted your nomination, we will contact the nominee with further information and details about the event.

Please note in order to complete the nomination form, due to GDPR, for young people aged 13+ we need you to gain their consent, or parent/carer consent if nominating young people aged 13 or under. This is consent to share their information such as their address and date of birth. Please ensure you have this information before nominating and that you get your nominations to us by Sunday 16th October!!
