Surrey Short Breaks survey 2015
Overview
Surrey Short Breaks for Disabled Children, part of Surrey County Council (SCC), funds a number of organisations and schools to provide ‘short breaks’ including but not limited to: after-school clubs, holiday schemes, help at home and overnight residential stays.
Our last parent carer survey took place in 2013 so we would like to find out whether your views have changed in the past two years. Your feedback will be used as evidence of the important role of short breaks in the lives of children with disabilities and their families.
This survey should take five to ten minutes to complete.
We don't routinely share individual responses, and we will not disclose names without permission. Anonymity cannot be guaranteed if a safeguarding concern is identified.
