Survey for carers of people with Dementia
Overview
Surrey County Council is currently reviewing training available for unpaid carers who are supporting someone with dementia. We would appreciate it if you could complete a short survey so that we can better promote access to information, training and advice for unpaid carers supporting someone with dementia.
Please do not share any additional information about your care and support on this form as we will not be able to respond. If you have any concerns or anything you would like to discuss, please speak to your social care team. Contact Action for carers to find out more.
