Questionnaire for patients who may use Surrey sexual health services

What is this questionnaire about?

We are looking to gain your feedback about sexual health and HIV services in Surrey. The services we are asking for feedback on are:

  • Central and North West London (CNWL) clinics, online and outreach services
  • Pharmacies (emergency hormonal contraception, chlamydia and gonorrhoea testing kits and chlamydia treatment)
  • Long Acting reversal contraception (LARC) (provided by GPs)

The questionnaire will be available to complete on line until the 3 May 2019. Your responses are very important to us and will be used to ensure that the services are meeting your needs.

Completion of this questionnaire is voluntary. You do not have to answer any questions you don’t want to and you do not have to give us a reason.

    We will be holding engagement events in partnership with Public Health Surrey County Council at:

    Woking Sexual Health Clinic, Woking Community Hospital, 2 Heathside Road, Woking, Surrey GU22 7HS

    This will be an opportunity for patients to share their views and experiences of the service they have received from CNWL. The events will be led by staff who work in the service who will be available to answer any questions, concerns and compliments you may want to share. There will be a paper based questionnaire that patients will be encouraged to complete and hand in

  • Untitled EPatient Engagement Event

    From 29 Apr 2019 at 15:30 to 29 Apr 2019 at 17:30

    We will be holding engagement events in partnership with Public Health Surrey County Council at:

    Earnsdale Sexual Health Clinic, 2 Whitepost Hill, Redhill, Surrey RH1 6BD

    This will be an opportunity for patients to share their views and experiences of the service they have received from CNWL. The events will be led by staff who work in the service who will be available to answer any questions, concerns and compliments you may want to share. There will be a paper based questionnaire that patients will be encouraged to complete and hand in.
