Questionnaire for professionals who may signpost to Surrey sexual health services

What is this questionnaire about?

We are looking to gain your feedback about sexual health and HIV services in Surrey. The services we are asking for feedback on are:

• Central and North West London (CNWL) clinics, online and outreach services in Surrey
• Pharmacies (emergency hormonal contraception, chlamydia and gonorrhoea testing kits and chlamydia treatment)
• In addition to this questionnaire we are also sending out a separate questionnaire to GPs regarding long acting reversal contraception provision (LARC).

This questionnaire for professionals is being carried out alongside a questionnaire for patients and users of the sexual health service as part of ongoing engagement.

The questionnaire will be available to complete on line until the 3 May 2019. Your responses are very important to us and will be used to ensure that the services are meeting the needs of your patients and clients.
