Education, Health and Care needs assessment (EHCNA) form and guidance - parent carer feedback form
Overview
Over the last few months, Surrey County Council (SCC) and Family Voice Surrey (FVS) have been working with parent carers to understand how the EHCNA process can be improved.
The most recent online discussion group on 10th June 2025, hosted by Family Voice Surrey and the SCC Special Education Needs and Disabilities (SEND) Team, focused more specifically on the EHCNA Parent Request Form and guidance for parent carers.
We would like anyone who registered for the group above to also have the opportunity to provide your views in written format here. This is so we hear from as many of you as possible, in case you weren't able to join on the day, or if you have had further suggestions you wanted to share.

Why your views matter
By getting involved you are helping the SCC SEND team to understand how their suggestions can be improved and representing what parent carers want from these parts of the EHCP process.
The form includes some open questions for your comments on 2 areas (each is optional, so if you only want to provide suggestions for one of them, please skip the remainder)
