NHS 10 Year Plan Primary Care Focus Group

Overview

Register your interest to take part in an online focus group to help shape the NHS 10-Year Plan and what it means for Surrey Heartlands.

The focus group will take place in January or February and you can select your preferred date and time in this form. The session will last approximately 2 hours and 30 minutes. Date and time will be confirmed depending on participants' availability.

Places are limited and selected participants will receive an information pack and consent form prior to the focus group session. This form will close on 3rd January 2025.

All the data you provide in this form will be held in confidence and your participation in the engagement will remain private.
