Multi-Professional Reference Panel Registration Form

Closed 30 Jun 2022

Opened 22 Feb 2019

Overview

Clinical and professional engagement and co-design are fundamental to all Surrey Heartlands programmes following academic principles of best practice. Going forward, there is a need for an approach that secures consistent, effective clinical engagement across the ICPs and the ICS to ensure sustainable, high quality co-design to transform services. The Multi-Professional Reference Panel is an opportunity for health and social care professionals to connect with each other, work together to address variation in practice, improve outcomes and develop services collaboratively to meet the needs of our citizens.

Why your views matter

In order to ensure effective Multi-Professional engagement, it is essential that a wide range of staff with expertise in various specialist interest areas register to be a part of this broader virtual Reference Panel. It is expected that further iterations will follow as the panel accumulates more members.

Once you register to be a part of this virtual Reference Panel, your details will be held securely in a database and accessed by the database manager for opportunities to participate. For example, you will have opportunities complete surveys or attend stakeholder workshops and more, for the purpose of redesigning pathways, reviewing best practice, identifying key issues, and implementation plans. In addition, you can also contribute ideas and innovation to be filtered up to the programme board.

Your details will be held for 12 months and after this time and you will be contacted to gain your permission to keep your details on this secure database. You may withdraw your details at any time without having to give a reason.
