Surrey Disability Register Data Protection Questionnaire

Overview

In line with the new General Data Protection Regulation coming in force in May 2018, we need to confirm that you still wish to be on the Surrey Disability Register, that your details are correct and that you still want to receive our newsletter.

To help us to do this please would you answer the following few brief questions confirming your personal details by 31 March 2018. 

We will send you a reminder in the July 2018 newsletter and if we haven’t heard back from you by the end of September 2018, we will assume that you no longer wish to be on the register and will remove you from our database.
